Please complete the prescription re-order form then post, fax or drop in to the surgery allowing 48 hours processing. You can either collect your prescription, enclose a SAE or we can send it to Larkhall Pharmacy, Batheaston Pharmacy or Boots.

The Doctors and Nurses regularly review medications and may ask you to come for an appointment for this reason.

Please note we do not accept telephone requests for repeat prescriptions as mistakes are possible.
